These terms explain what Future Self does, what independent clinicians and pharmacies do, and what you agree to when using the Service.

Acceptance of these Terms
These Terms of Service (“Terms”) are a binding agreement between you and Superstar Wellness, operating as Future Self RX (“Future Self,” “we,” “us,” or “our”), and govern your access to and use of futureselfrx.ai, our web and mobile experiences, AI concierge, iMessage features, quizzes, and related services (collectively, the “Service”).
By creating an account, using the Service, or completing a purchase, you agree to these Terms and our Privacy Policy, Medical Disclaimer, and, when applicable, Telehealth Consent. If you do not agree, do not use the Service.
Future Self is technology and coordination
Future Self provides health education, intake organization, personal record tools, care navigation, price and option information, administrative support, reminders, and an AI concierge. At your direction, we may help transmit information to and coordinate services with independent licensed clinicians, pharmacies, laboratories, and fulfillment providers (“Third-Party Providers”).
Future Self may act as your administrative or purchasing agent by collecting an authorized payment and using the applicable portion to arrange and pay for Third-Party Provider services or products on your behalf. Collecting payment does not make Future Self the provider, prescriber, pharmacy, laboratory, manufacturer, or dispenser.
Not medical care or emergency care
Future Self is not a medical practice, healthcare provider, pharmacy, prescriber, manufacturer, or laboratory. Future Self does not diagnose, prescribe, dispense, or independently decide whether a treatment is appropriate. Using the Service does not create a clinician–patient or pharmacist–patient relationship with Future Self.
Any clinical relationship is solely between you and the independent licensed clinician who reviews your information. Clinicians exercise independent medical judgment. Pharmacies independently perform dispensing and related duties.
The AI concierge is an educational and administrative tool. It may be incomplete, inaccurate, or unsuitable for your circumstances and must not be relied on for diagnosis, dosing, treatment selection, or emergency decisions. Always seek advice from a qualified licensed clinician.
Your account and responsibilities
You must be at least 18, able to form a binding contract, and located in a U.S. jurisdiction where the Service and requested services are available. You may use the Service only for yourself unless we expressly support another authorized use.
You agree to provide accurate, complete, and current information—especially information a clinician may rely upon—and to update it when circumstances change. You are responsible for maintaining the confidentiality of your account and for promptly notifying us of suspected unauthorized access.
We may suspend or terminate access where reasonably necessary to protect safety or security, investigate fraud or misuse, comply with law, or enforce these Terms.
Prices, saved cards, orders, and refunds
The amount displayed before purchase may include Future Self technology, concierge, care-coordination, and administrative fees, plus amounts used to arrange clinical review, laboratory services, pharmacy fulfillment, products, shipping, or other Third-Party Provider services. The checkout or order confirmation will describe the amount you authorize.
Saving a payment method does not authorize a charge. A saved card may be charged only after you explicitly approve a specific purchase and amount. Payment data is processed through Stripe; Future Self does not receive your full card number or security code.
Payment may be collected before independent clinical review. Payment is not a promise of approval, prescription, dispensing, or a particular outcome. If an independent clinician does not approve a prescription order, the corresponding order will be cancelled and refunded as described at checkout or in the order confirmation. Once a prescription product has been dispensed specifically for you, it generally cannot be returned or resold.
If a subscription or recurring service is offered, the price, frequency, renewal terms, and cancellation method will be shown before enrollment. You may cancel future renewals as disclosed at checkout; cancellation does not retroactively reverse a completed period or fulfilled order unless stated otherwise or required by law.
Independent providers, products, and integrations
Third-Party Providers are independent and are not controlled by Future Self. Future Self does not direct clinical judgment or pharmacy dispensing. Providers may have separate terms, consents, privacy notices, eligibility rules, and record-retention duties.
Subject to your authorization and applicable law, Future Self may coordinate with or replace a Third-Party Provider based on licensure, availability, service level, price, or product availability. We will seek your approval before a substitution that materially changes the product or total price.
Some products discussed through the Service may be compounded or may not be FDA-approved for the use being discussed. Compounded drugs are not FDA-approved, and the FDA does not verify their safety, effectiveness, or quality before marketing. Outcomes vary; no result is guaranteed. Product-specific risks and suitability must be reviewed with the licensed clinician and dispensing pharmacy.
Optional face-scan estimates, wellness scores, connected-device readings, and similar features are not medical devices or diagnostic measurements unless expressly stated. Do not use them for emergency or treatment decisions.
Messages, reminders, and electronic records
By providing contact information, you consent to transactional, authentication, safety, service, and care-coordination messages. Where you separately opt in, you may also receive informational or marketing communications. Message and data rates may apply. Reply STOP to opt out of texts and HELP for help. Marketing consent is not a condition of purchase.
You agree to receive agreements, notices, disclosures, and records electronically. You are responsible for keeping your contact information current.
Acceptable use and intellectual property
You may not use the Service unlawfully; submit false or misleading health information; resell or distribute prescription products; impersonate another person; access another person’s account; probe, scrape, reverse engineer, overload, or disrupt the Service; introduce malware; evade security controls; or use the Service or its output to build a competing product in violation of law or our rights.
The Service, software, content, design, branding, and related intellectual property are owned by Future Self or its licensors. We grant you a limited, revocable, non-exclusive, non-transferable license to use the Service for personal, non-commercial purposes in accordance with these Terms.
You retain rights in content you provide. You grant us the limited rights needed to host, process, transmit, and use that content to operate, secure, support, and improve the Service and provide features you request, subject to our Privacy Policy.
Disclaimers and limitation of liability
TO THE FULLEST EXTENT PERMITTED BY LAW, THE SERVICE AND CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, EXPRESS, IMPLIED, OR STATUTORY, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, ACCURACY, OR UNINTERRUPTED AVAILABILITY. FUTURE SELF DOES NOT WARRANT ANY THIRD-PARTY PROVIDER, PRODUCT, OR CLINICAL OUTCOME.
You understand that medications, peptides, compounded products, laboratory tests, and other health services may involve risks. Decisions about them belong to you and the independent licensed clinician. Nothing in these Terms limits rights or liability that cannot legally be limited.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, FUTURE SELF AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES; LOSS OF PROFITS, DATA, OR GOODWILL; OR ACTS OR OMISSIONS OF THIRD-PARTY PROVIDERS. FUTURE SELF’S TOTAL AGGREGATE LIABILITY FOR CLAIMS RELATING TO THE SERVICE WILL NOT EXCEED THE GREATER OF THE AMOUNT YOU PAID FUTURE SELF DURING THE SIX MONTHS BEFORE THE EVENT GIVING RISE TO THE CLAIM OR $100. SOME JURISDICTIONS DO NOT ALLOW CERTAIN LIMITATIONS, SO SOME LIMITS MAY NOT APPLY.
Individual arbitration and class-action waiver
You and Future Self agree that disputes arising from these Terms or the Service will be resolved by final and binding individual arbitration administered by the American Arbitration Association under its applicable Consumer Arbitration Rules, rather than in court, except that either party may bring an eligible individual claim in small-claims court or seek appropriate relief for intellectual-property misuse.
You and Future Self waive the right to a jury trial and to participate in a class, collective, consolidated, or representative action. The Federal Arbitration Act governs this section. Unless the parties agree otherwise, arbitration will be conducted remotely or at a reasonably convenient location.
You may opt out of this arbitration agreement within 30 days after first accepting these Terms by emailing care@futureselfrx.ai with your full name, the email or phone number associated with your account, and a clear statement that you opt out of arbitration. Opting out does not affect any other part of these Terms.
Before initiating arbitration, the complaining party must send a written notice describing the dispute and requested relief. The parties will attempt in good faith to resolve it informally for 30 days. Notices to Future Self may be sent to the support email above.
Changes, governing law, and contact
These Terms are governed by Delaware law, without regard to conflict-of-law principles, except where federal law applies or another jurisdiction’s non-waivable law controls. For disputes not subject to arbitration, the parties consent to state or federal courts in Delaware, unless applicable law requires otherwise.
We may update these Terms as the Service or law changes. We will post revised Terms with a new effective date and provide additional notice where required. Material changes apply prospectively unless otherwise permitted by law.
These Terms and incorporated policies are the entire agreement about the Service. If a provision is unenforceable, the remaining provisions remain effective. Failure to enforce a provision is not a waiver. You may not assign these Terms without our consent; we may assign them in connection with a business transaction. We are not responsible for delays caused by events beyond reasonable control.
