This consent describes how telehealth may work when you request clinical services coordinated through Future Self RX. You accept it when you affirmatively check the telehealth-consent box during checkout or another consent flow.
How remote clinical review works
When you request a prescription product or clinical service, Future Self may organize and transmit the health intake you provide—including medical history, medications, allergies, and related information—to an independent clinician licensed where you are located.
Future Self is not the clinician or medical provider and does not direct clinical judgment. Review may be asynchronous, meaning there may be no live video or phone visit, only where the clinician considers that format appropriate and applicable law permits it. The clinician may ask follow-up questions or require a live or in-person evaluation.
Payment does not guarantee approval
Future Self may collect the total displayed at checkout before clinical review. That amount may include Future Self technology, concierge, and coordination services and amounts used to arrange independent clinical, pharmacy, laboratory, shipping, and fulfillment services you authorize.
Payment does not create a provider-patient relationship with Future Self and does not cause a prescription to be issued automatically. The price, cancellation terms, and any refund terms shown for the specific order apply.
The clinician may approve, decline, or request more information
- Approved: an independent licensed clinician may authorize a prescription, after which an appropriately licensed pharmacy may dispense it subject to pharmacy review, availability, and applicable law.
- More information needed: the clinician may ask questions, request records or testing, or require a live or in-person evaluation before deciding.
- Not approved: no prescription is issued. Cancellation and refund handling follow the terms disclosed for the specific order and any applicable clinician, pharmacy, or fulfillment policy.
The clinician’s prescribing decision is independent of Future Self.
Telehealth has practical benefits and important limits

Telehealth can make licensed clinical review more convenient and may reduce travel or appointment burden.
Potential risks and limits
- The clinician relies heavily on the accuracy and completeness of the information you provide.
- Without an in-person examination, some findings may be unavailable or less certain.
- Technology failures, incomplete records, or delayed messages may delay evaluation or coordination.
- No electronic system can promise perfect privacy or security.
- Telehealth may not be appropriate for every condition, product, location, or person.
Provide accurate information and follow instructions
- Provide complete, accurate, and current health information and promptly update material changes.
- Be physically located in the state you identify at the time of the clinical service.
- Use any prescribed product only as directed by the prescribing clinician and dispensing pharmacy.
- Maintain appropriate in-person and ongoing healthcare relationships and tell your healthcare team what you are taking.
- Contact the clinician or pharmacy promptly about side effects, dosing questions, or unexpected symptoms.
You have choices
- You may decline telehealth and may withdraw consent before the relevant review is completed, subject to the cancellation terms disclosed for the specific order.
- You may ask questions about the telehealth process and request information about the independent clinician.
- You may request available records of your telehealth encounter, subject to applicable law and provider record policies.
- Your information is handled as described in our Privacy Policy, the independent provider’s privacy notice, and applicable law.
- You may seek in-person care or another provider at any time.
Services vary by state and circumstance
Telehealth and prescription services are available only where the applicable clinical and pharmacy networks operate and where the requested service or product is legally and clinically available. Availability may vary by state, product, compound, age, medical history, and clinician judgment.
What you confirm when you consent
By affirmatively accepting this consent, you confirm that you have read and understood it; have had an opportunity to ask questions; will provide accurate information; voluntarily consent to telehealth services from an independent provider as described; and authorize Future Self to coordinate the administrative, clinical, pharmacy, and fulfillment services associated with the specific request you approve.
